Yooralla's new School Holiday Program for teenagers (aged 12 to 18 years)

Yooralla's Recreation Service has recently won a tender to run a School Holiday Program for teenagers (aged 12 to 18 years) residing in the Hobson's Bay Area who have high and complex support needs.
Below is a copy of a referral form for your reference. Please note that to be eligible for this service, teenagers must:

Be between the ages of 12 to 18 years
Have high and complex support needs
Reside in the Hobson's Bay area
Live with a primary carer
